What Is CHED Scholarship Form?

The Commission of Higher Education (CHED Philippines) offers a scholarship program for learners who are financially weak to have access to quality education. CHED scholarship is specially devised to help high students get through the financial troubles during the times of the COVID-19 pandemic and join the college they want. Who Can Apply for a CHED Scholarship?

To enroll in the CHED scholarship program, there are certain qualifications and eligibility criteria that need to be fulfilled. According to the Philippines government, the qualification requirement includes:

  • For the complete scholarship, a grade of a minimum of 90% is expected.
  • For partial scholarship, a grade of 85% of the above is needed.
  • The applicant should be a high school graduate.
  • The total income of the applicant's parents or guardians should not be more than P300,000.00.
  • The student must not be a degree holder.

The CHED scholarship form consists of various information that helps the authorities to check the genuineness of the applicants requesting the scholarship. The form needs to be filled with the following details and sent to the Philippines Commission on Higher Education Regional Office.

  • Personal Information - The personal details of the applicant are included here. This comprises name, date & place of birth, civil status, contact information, address details, the particulars of the applicant's school education, and specifications about any disabilities the applicant has.
  • Family Background - The complete details of the applicant's parents or guardian is required. You need to fill in their name, address, occupation, educational qualification, and total gross income. Siblings’ details are also to be entered in this area.
  • Enrolling School Details - Give information about the school you want to enroll in with the scholarship. Name, address, and type of school are to be mentioned. The course you wish to join in the school.
  • Photograph - Photographs of the applicant need to be attached to verify the identity.
  • Signature - Date of filling the application and signature of the applicant declaring that all the information provided is correct.
  • Information to Be Filled by CHED - This section includes details to be filled by the CHED officials like what scholarship the applicant will be qualified for if the person comes under any special categories, documents attached with the form, and signature of the processing officer.

Documents Needed to Apply for CHED Scholarship

Below are the documentary requirements for applying for the CHED scholarship.

  • For high school students in the fourth year - Grade documents of the first 3 years and the completed grading periods of the fourth year.
  • For applicants who have graduated from high school - Form 138 indicates the grades of the student depicting the person's promotional status.
  • For applicants in college - Grade certificate for the last two semesters.
  • For others - An Accreditation and Equivalent Test Passer Certificate (ALS) and Certificate of advancing to the next level (PEPT) are needed.
  • Good Moral certificate from the previous school I studied.
  • Any one of the forms from the following that shows the income range of the family: Parent or guardian's latest income tax return, tax exemption certificate, indigency certificate, or affidavit of no income.
